Cognitive Neurology Patients

Process for New Patients

Our goal is to determine if cognitive symptoms are present and identify the best path forward for diagnosis, management, and timely care.

  • Step 1: Complete digital or technician-administered cognitive screen (memory tests and questionnaires). Click here to schedule your cognitive screen or call 404-778-3444.
  • Step 2: Our cognitive team will complete an expert review of the cognitive screen and questionnaires.
    • Questionnaires will be available in MyChart seven days prior to the cognitive screening appointment. Complete these in MyChart prior to your cognitive screening appointment.
  • Step 3: A report will be sent via MyChart within 3 days of your cognitive screen with the best path forward.
    • The cognitive screen may recommend a visit with an Emory Cognitive Neurology provider for additional evaluation; however, an alternative path may be recommended to ensure timely care:
      • A visit to one of our partner clinics
      • Returning to care of referring specialist or PCP
      • Connection to other resources

Follow-Up Appointments

Some patients come for a one-time evaluation or second opinion. If follow-up care is desired, the appointment frequency is based on clinical need and availability. Appointments may be conducted in person or via telemedicine approximately every 12 months. This reflects the expectation that change in these conditions will be gradual and not require frequent observation. If you have a new concern, please contact us via MyChart and we will work to meet the needs.

Scope of Care

We utilize diagnostic tests, prescribe medications, and provide care planning advice focused on cognitive conditions and symptoms. We do not manage other medical conditions. We recommend that you work closely with your primary care provider or other specialists to ensure your overall health.

Communication

Communication with your provider is best managed by messaging from MyChart. Logging in to MyChart through your phone app or website gives you access to clinic notes, test results, and a direct line of communication to your care teams. Please allow at least a 48-hour response time.
