Your health record includes your medical history. It lists information such as allergies, medicines, and test results. It also has your health insurance information and how to contact you.
Our duty at Emory Healthcare is to ensure that your medical record remains confidential. We maintain and release it according to specific laws. You can request access to your patient health information in several ways.
The online tool verifies your identity by requesting a photo of your driver's license. You can use your smartphone or a webcam to take the photo.

You can have a copy of your medical records sent to another medical facility. You also can get a paper copy for yourself.
Step 1: Download and fill out the Emory Healthcare Authorization for the Release of Protected Health Information form.
Step 2: Fax, mail or email the form to the Medical Records/Health Information Management Department at the Emory Healthcare facility where you received treatment. You can find contact information for each facility below.
There is no charge to have your medical records sent to another medical facility. If you want your own paper copies, there is a minimal fee per page. You can see these fees on the Authorization for the Release of Protected Health Information Form.
