Accessing Your Medical Records

 

Your health record includes your medical history. It lists information such as allergies, medicines, and test results. It also has your health insurance information and how to contact you.

Our duty at Emory Healthcare's Health Information Management (HIM) department is to ensure that your medical record remains confidential. We maintain and release it according to specific laws. You can request access to your patient health information in several ways. 

Emory Warner Robins and Emory Perry (formerly Houston Healthcare) are now a part of Emory Healthcare. To obtain clinic records, please submit your request via the instructions on this website. For hospital records, please submit your request via this link/URL: https://www.hhc.org/patients-visitors/request-medical-records/.

Electronic Request for Records

Emory Healthcare offers an online patient record request tool you can use for free. Use this tool for:

  • Requesting your own records
  • Parents of minor patients requesting records
  • Caregivers acting on a patient’s behalf (Power of Attorney)

The online tool verifies your identity by requesting a photo of your driver's license. You can use your smartphone or a webcam to take the photo.

Request Records

Paper Request for Records

You can have a copy of your medical records sent to another medical facility. You also can get a paper copy for yourself. 

Step 1

Download and complete the Emory Healthcare Authorization for the Release of Protected Health Information form. The package includes the ROI Form, Patient Fee Sheet, and instructions for submitting an E-Delivery Records Request.

Step 2

Fax, mail or email the form to the Medical Records/Health Information Management Department at the Emory Healthcare facility where you received treatment. You can find contact information for each facility below.

There is no charge to have your medical records sent to another medical facility. Please see Patient Fee Sheet, which is the 4th page of the authorization packet.

Medical professionals who need records can use our paperless request process. It does not require a login and uses email verification for enhanced security.

How to Request to Amend Your Medical Record

You can ask to amend the information in your medical record. This can help improve the accuracy of your information, and your record will also retain the original information.

 

Once the amendment form has been completed, it can be emailed to: patientamendments@emoryhealthcare.org.
