Medical Records FAQs

It generally takes 7-10 business days after we receive your request. But it may take longer if the doctor or medical professional needs to update the record.

Yes. The fee calculation is per page. The federal government sets the fees. However, some state fees are lower.

Information on fees is on the last page of this Release of Information form.

No. There is no fee if a record request is for continued patient care, but there are record request fees for:

• Attorneys
• Insurance companies
• Organizations for workers’ compensation or disability
• Personal use

The Emory Clinic re-releases outside records for all record request types.

Emory Hospitals re-release records for legal requests only.

Emory Johns Creek releases outside records if they will support treatment.

No. Once we receive your request at one facility, we will forward it to the other Emory facility(s) where you are requesting records.

You are responsible, however, for following up. Each facility processes its own records requests.

 

For personal requests,  our office hours of operation are 8:30 a.m. to 5 p.m., Monday through Friday. We are not open on weekends. Select locations have modified hours of operation: 

• Emory Johns Creek hours are 8 a.m. to 5 p.m., Monday through Friday.

• Emory Decatur Hospital, Emory Long Term Acute Care and Emory Hillandale Hospital hours are 8:30 a.m. to 6 p.m., Monday through Friday.

For requests from medical professionals, we process requests for records 24 hours a day, seven days a week for immediate patient care.

 

 

Yes. Contact the HIM/Medical Records department to schedule an onsite appointment. You can also view test results and other medical information in your MyChart patient portal. 

The Release of Information and HIM/Medical Records staff cannot answer clinical questions. If you have medical questions, please contact your care team through your MyChart patient portal or call their office directly.

 

The authorization form expires 90 days from the date you sign it, or on the specific date you list on the form.
