In-Hospital and Post-Hospitalization Resources
A care manager, who may be either a social worker or a registered nurse, can provide information about hospital and community resources. Our Care Management Services team can also arrange for:
- Alternate living arrangements: When you need services that aren’t available in your home, you may qualify for alternate living arrangements. Please note that patients must meet certain clinical requirements for placement and have a payer source or pay out-of-pocket.
- Information for you and your doctor about:
- Financial arrangements
- Hospital policies
- Insurance coverage
- Other issues
- Referrals to community agencies:
- Financial assistance
- Various public and private groups
The following services are offered for patients who have specific health needs, payer authorization and reside in a geographical area where these are provided.
- Short-term support services at home, including:
- A companion
- A visiting nurse or aide
- Meals on Wheels
- Medical equipment and supplies
- Medical social services
- Physical, occupational, or speech therapy
- Transportation
- Short-term nursing home care: To prepare you to live independently at home
- Supportive counseling: To help you cope with changes in your life due to your illness
Patients can contact their care manager about these services if they have not received them.
Discharge Planning and Post-Hospital Care
Before you leave the hospital, a care manager will meet with you and your health care team to discuss your clinical care needs and go over your discharge options.
