Emory Healthcare utilizes the services and technology from Club Automation (A Daxko, LLC. Company) to provide the Emory Decatur Wellness Center mobile app. Collectively, Emory Healthcare and Club Automation (Daxko, LLC.) ("us", "we", or "our") operate, manage the content, and support the mobile app and related websites.

Personal Information Collection, Use, Disclosure, and Consent
Personal Information may include your name, age, home address, phone number, email address, or any information you are prompted to provide to use the dashboard or mobile app platform.

Consent may also be implied where you have notice and a reasonable opportunity to opt out, such as by not using the platform or mobile app. We may collect, use, or disclose Personal Information without the consent: When required by law; In an emergency that threatens an individual's life, health, or personal security; When the Personal Information is available from a public source; Other legally established reasons.

We will not sell your Personal Information to other parties. We retain your Personal Information for the time necessary to fulfill the identified purposes or a legal or business purpose. Appendix A: App Permissions
We are committed to protecting both your privacy and anonymity in the use of a mobile application.

Understanding App Permissions
Occasionally, we or our organizational partners receive questions from users on why the app requires so many different permissions. To that end, we've listed out the various permissions so you can understand why they are needed for your custom mobile app.

Identity Permissions
We do not capture any personally identifiable information without your consent. Only the generic device ID is gathered so that your download of the app can be associated with your Google Play/AppStore/Blackberry World account. The App Store company takes that information so they can track your downloads from their App Store. Also, if you choose to subscribe to push notifications, your device ID is used to associate with your phone to receive the notifications.

WiFi Connection Information Permissions
The system admins at your organization can send mass notifications called "push notifications". Push notifications go over the WiFi network, so the app simply looks and sees if you're connected to WiFi so it can know if you'll receive that message or not. As well, the app periodically updates with new information and content - this also requires that WiFi connection.

Device ID and Call Information Permissions
The Device ID is an identifying number for your device. Most apps require this so they can "tie" the app to your phone. The call information is only available in the scenario where you try to use the contacts to make a call. The app will "auto-populate" the call information field with the club’s number; that's why it requires the call information access.

Conclusion
Nothing happens unless the user consents to it. If, after all this, you still feel uneasy, you don't have to download the app. While you have no reason to be concerned, we can respect that decision as well.
